I write on behalf of the ________________ program at Baptist Bible College & Seminary to request permission to duplicate and distribute or display a work (or, portion of a work). It is my understanding that you (your company) own(s) the copyright to the work we want to duplicate. The description of the work and our intended use of it is as follows:

(If we are talking about a photograph where the name of the photographer is not apparent, describe exactly where it appears.
